About The Position

AHN Healthcare@Home is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. This is a Weekend Registered Nurse (RN) position for a Home Health role, providing skilled nursing care to patients in their homes in the Greensburg, PA area. This position is eligible for a $10,000 first year success bonus, with bonus opportunities extending beyond the first year. The role involves delivering skilled, patient-centered care in a home health setting, collaborating with an interdisciplinary team, performing patient assessments, medication management, and care planning. It also includes educating and empowering patients and families, critical thinking, real-time problem-solving, compliant EMR documentation, and traveling to patient homes within an assigned territory.

Requirements

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
