Requirements

Degree from an accredited school of registered nursing (Associate’s or Bachelor’s degree)
Current Massachusetts Registered Nurse (RN) license in good standing
Strong clinical assessment and patient care skills
Ability to work effectively in a fast-paced, team-based healthcare environment

Nice To Haves

Bachelor of Science in Nursing (BSN) preferred
Experience in a primary care, outpatient, or community health setting
Experience working with diverse or underserved populations
Bilingual skills preferred but not required
Strong critical thinking and clinical decision-making skills
Ability to prioritize and manage multiple patient needs effectively
Excellent communication and patient education skills
Strong teamwork and collaboration skills

Responsibilities

Provide comprehensive nursing care using all phases of the nursing process, including assessment, planning, implementation, and evaluation
Assess patients through both phone triage and in-person visits using established clinical protocols
Collaborate with primary care providers to deliver appropriate patient care and coordinate escalation when needed
Work as part of the primary care team to manage patients from diverse age, cultural, and socioeconomic backgrounds
Involve patients, families, and appropriate healthcare providers in care planning when appropriate
Provide patient and family education based on individual needs, health literacy, and cultural considerations
Communicate clearly, concisely, and professionally with patients and care team members in both written and verbal formats
Maintain timely, accurate, and complete documentation in the electronic health record (EHR)
Work closely with medical assistants and administrative staff to ensure efficient coordination of care, scheduling, prescriptions, and paperwork
Demonstrate clinical and technical competence, including phlebotomy, point-of-care testing, medication administration, vaccinations, and injections
Administer medications safely using the “five rights” of medication administration
Maintain a safe and clean clinical environment through adherence to infection control and risk management practices
Oversee and support clinical workflows and delegate tasks appropriately to Medical Assistants and other support staff
Perform effectively and professionally in a fast-paced environment and during high-demand situations
Ensure compliance with all SBCHC policies, procedures, and regulatory standards, including completion of mandatory trainings
Participate in ongoing professional development and training opportunities
Perform additional duties as assigned
