About The Position
Nightingale Visiting Nurses is hiring two full-time Registered Nurses for a territory-based home health role serving Stamford, Norwalk, and nearby areas in Connecticut. The position offers a salary with performance-bonus potential and is not paid per visit. Nurses manage a defined patient population with clinical autonomy and organizational support, providing care in patients’ homes. The role includes a consistent patient volume, mileage reimbursement, paid time off, benefits, and access to leadership. Top performers may have the opportunity to grow into clinical leadership roles as the office expands.
Requirements
- Active Registered Nurse license in Connecticut.
- Home health experience is required.
- Strong clinical judgment and ability to work independently.
- Reliable transportation.
- Commitment to high-quality patient care.
Nice To Haves
- OASIS experience is strongly preferred.
Responsibilities
- Manage a defined patient population in Stamford and nearby areas.
- Perform Start of Care (SOC), recertification, and routine visits.
- Complete OASIS assessments and develop Plans of Care.
- Monitor patient progress and adjust care accordingly.
- Coordinate care with therapists, aides, and physicians.
- Complete timely, accurate documentation in WellSky.
- Deliver high-quality, patient-centered care in the home setting.
