About The Position

This position provides professional wound care treatment to patients at the Wound Care Clinic, utilizing established policies and procedures, ensuring safe and effective care. The scope of care varies based on the levels of complexity. Services are provided based on the ages of the population served; adolescents through geriatric - 65 years or older and in collaboration with the multidisciplinary care team. This RN assesses, plans, implements and evaluates care in collaboration with the care team. Demonstrates the knowledge and skill necessary to identify and meet the individualized needs of all patients served in the Wound Care Clinic.

Requirements

  • Completion of an accredited Nursing program
  • RN Wound Care Certification Course within 6 months of hire
  • Bachelors of Science in Nursing BSN required within 3 years of hire
  • Two (2) years minimum nursing experience required
  • Knowledge of nursing practices in wound care
  • Highly effective written and verbal communications skills
  • Proficient computer skills as are required to document, communicate and enter information into the electronic medical records system
  • Requires critical thinking skills, decisive judgment and ability to work with minimal supervision
  • Must be able to work in a stressful environment and take appropriate action
  • Ability to collaborate with various departments in and outside of the Barton Health System in order to provide consistently exceptional patient care
  • In compliance with patient safety standards, must be able to effectively communicate in English
  • Current California RN license or ability to obtain prior to start date.
  • Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date.

  • Bilingual abilities preferred
  • Certification in wound, continence and/or ostomy (CWN, CWON, CON, CCN, COCN, CWCN, WCC, CFCN or CWS) and or foot and nail services (CFNC is required within 3.5 years of hire.

Responsibilities

  • Provides consistently exceptional care at all times.
  • Coordinates treatment to provide comprehensive wound assessment and plan of care, including goals, implementation of treatment, evaluation and outcomes for patients through the Wound Care Clinic.
  • Provides patient, family and staff education regarding wound management, prevention of infection, and dressing replacement.
  • Develops skin care programs to prevent skin breakdown of patients.
  • Evaluates outcomes, consults with healthcare team members as required and ensures adjustment of patient care plans to secure optimal care.
  • Ensures that appropriate nursing actions are initiated on patients based on assessment findings.
  • Recognizes unusual conditions outside of their expertise and seeks guidance of colleagues or supervisors.
  • Assists healthcare providers with examinations, procedures and other processes related to direct patient care.
  • Monitors and ensures accuracy of documentation in patient medical record. All aspects of patient care are accurately and completely documented.
  • Operates equipment and performs job related duties in a safe manner. Ensures proper functioning of equipment and follows procedure when equipment malfunctions.
  • Actively participates in Shared Governance.
  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served.
  • Demonstrates a working knowledge of regulatory and licensing standards and ensures that practice is compliant with standards.
  • Responds to the needs of the department by performing other duties, as necessary.
