About The Position

CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties, offering comprehensive care including medical, dental, behavioral health, and substance use treatment. The Welcome Baby: Road to Resilience (WB:R2R) Registered Nurse (RN) provides in-home nursing care to postpartum Yolo County residents and their babies. This includes physical assessments, screenings, education, and connection to primary healthcare, behavioral healthcare, and community resources. The RN also provides telephone follow-up for clients as needed. The position requires the RN to be trained as a primary care RN with specialty training for home visits and may fill-in as a primary care RN at CCOLE when assigned. The RN works collaboratively with the perinatal health coordinator, other WB:R2R staff, and CommuniCare+OLE clinic managers, providers, and support staff to ensure quality, coordinated care.

Requirements

  • Completion of an accredited RN Program
  • Current California RN license and good standing with the Board of Registered Nurses required
  • CPR Certification required
  • At least one year of experience working with perinatal populations required.
  • At least one year of experience working with low-income populations in a clinic setting strongly preferred.
  • Current lactation certification (CLC/CLE or higher) and/or ability to achieve lactation certification within 6 months of hire.
  • BLS Certificate Required.
  • Drivers License Required.

Responsibilities

  • Provides and manages patient care for WB:R2R clients in their homes under the direction of the Associate Director of Maternal, Child, Sexual and Reproductive Health Services and staff medical providers, including physical evaluations, assessments, and diagnoses as appropriate for WB:R2R clients and their baby(s).
  • Consults with staff medical providers as needed for clinical concerns; accepts and correctly transcribes verbal or telephone orders from clinicians.
  • Provides and manages patient care for CommuniCare+OLE primary care patients in clinic settings under the direction of staff medical providers and the CommuniCare+OLE Director of Nursing.
  • Utilizes the nursing process (assessment, diagnosis, planning, intervention and evaluation) in patient care and daily activities.
  • Provides patient education to WB:R2R clients on essential topics, including but not limited to infant care (feeding, sleeping, cues, bathing, umbilical cord care, etc.), immunizations, postpartum mood and anxiety disorders, sexual activity and birth control, when to call a medical provider, and babywearing.
  • Participates in lactation training (as needed) and maintains lactation certification throughout employment. Provides lactation assistance to WB:R2R clients. Partners with CommuniCare+OLE IBCLC and other resources to provide ongoing lactation support.
  • Schedules WB:R2R clients for postpartum and well-baby follow up appointments.
  • Processes clinical patient information and WB:R2R data collection requirements in patient electronic health records (EHR).
  • Participates in quality improvement projects as assigned by perinatal health coordinator and/or Associate Director of Maternal, Child, Sexual and Reproductive Health Services.
  • Assists with coordination of specialty care for WB:R2R clients as needed.
  • Assists with meeting the needs of patients with complex medical problems, including facilitating referrals and follow-up care as needed.
  • Assists WB:R2R clients with coordination of care to other healthcare services, including dental care, behavioral health, diabetes education, etc.
  • Performs independent face-to-face or telephone triage using written triage protocols, for current/past WB:R2R clients and CCOLE patients with concerns related to postpartum and newborn care.
  • Attends WB:R2R, clinic and general CommuniCare+OLE meetings as required.
  • Performs other duties as assigned.
