About The Position

This position is for a Registered Nurse in the Cardiology department within the Medical Group. The role involves utilizing the nursing process to deliver patient care, including assessment, planning, implementation, and evaluation. The nurse will work collaboratively within a multidisciplinary team to coordinate care, monitor patient needs, and ensure desired outcomes are achieved. The position emphasizes patient experience, patient education on disease management, emergency care, drug information, and identifying community resources for patient self-management. The nurse will also be responsible for communicating the plan of care to patients and families, documenting patient health data, and adhering to organizational philosophies, objectives, policies, and procedures. Maintaining practice standards, clinical competencies, physician relationships, and peer relationships are also key aspects of this role. The nurse will participate in quality improvement activities and utilize supplies and equipment effectively and cost-efficiently.

Requirements

  • Successful completion of an Associate’s degree, diploma, or BSN from an accredited school of nursing required
  • Current licensure to practice nursing in the State of Illinois.
  • Current CPR certification required.
  • One year of nursing experience in an acute care setting preferred for most department specific positions.
  • Specialty experience requirements may vary based on practice.

Responsibilities

  • Utilizes the nursing process in the delivery of patient care including assessment, planning, implementation, and evaluation of patient outcomes revising plans of care as indicated.
  • Works collaboratively through a multidisciplinary team approach to facilitate plan of care, implements and coordinates care progression, monitors and evaluates options and services to meet an individual’s health needs.
  • Utilizes the nursing process to provide coordinated patient care in order to achieve desired outcomes as an active member of a multidisciplinary team.
  • Coordinates and provides holistic care based on the patient's individual needs in concert with the patient's family and/or significant other.
  • Demonstrates the knowledge and skills necessary to assess, treat, and provide care appropriate to the age(s) of the patients served.
  • May delegate patient care activities as appropriate.
  • Supports the NCH MG Physician in patient flow, patient education on disease, management strategies, emergency care and drug information when appropriate.
  • Identifies community resources to support patient self-management of disease.
  • Emphasis on patient experience, closing visits, delivering difficult results, triage and corresponding care instructions.
  • Coordinates care with provider and interdisciplinary team within NCH MG and with external resources.
  • Communicates and coordinates plan of care to patient and family.
  • Documents collection of patient health data, interventions and communications.
  • Supports and interprets Northwest Community Healthcare and the Medical Group department philosophies, objectives, policies, and procedures and applies them to all aspects of patient care.
  • Maintains practice standards and clinical competencies as established by the organization and regulatory bodies.
  • Provides direct patient care activities, procedures and protocols specific to the department assigned.
  • Maintains physician relationships that demonstrate mutual respect, trust, and open dialogue for the provision of quality patient care.
  • Maintains peer relationships that demonstrate support, collaboration, and goal-directed interaction.
  • Participates in quality measurement and improvement activities.
  • Utilizes supplies and equipment in the provision of patient care in a manner, which assures quality, cost effective patient care.
  • Adheres to all Northwest Community Healthcare standards, policies, and procedures.
