About The Position

Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients. This role performs a wide variety of training and administrative and management activities to manage the Home and Community Based Alternatives (HCBA) program.

Requirements

Valid RN license to practice in California.
A current California Driver’s License required.
Current automobile insurance.
Have a reliable vehicle.
Able to drive to destination, including HHCM offices in Sacramento and Redding, or client homes to facilitate training of new staff.
No driving and weather limitations.
Produce a clean Department of Motor Vehicles (DMV) printout prior to being hired.
One year of experience working as an RN.
Experience in discharge planning, home health, public health, skilled nursing, hospice or hospital setting or less than one year experience with a Masters Degree in Nursing with a Case management focus.
Knowledge of basic problems and needs of people diagnosed with a physical disability or emotional disturbance.
Knowledge of community and supportive services.
Skilled in assessing and prioritizing multiple tasks, projects, and demands.
Skilled in working within deadlines to complete projects and assignments.
Skilled in establishing and maintaining effective working relations with co-workers and other community providers.
Skilled in operating a computer utilizing a variety of standard software (including an electronic medical record desirable).
Excellent written and verbal communication skills.
Ability to work independently as well as in a team environment.
Flexible in adjusting to ever changing priorities.

Nice To Haves

Nationally Certified Case Manager or eligible to take CCM exam.
Prior experience working as a mentor for nursing students or new graduates or new employees.

Responsibilities

Participate in the onboarding process of new clinical staff, which includes planning and facilitating orientation.
Train new staff on creating, reviewing, signing, and uploading Plan of Treatments (POTs), for clients that are enrolled in the HCBA program. POT should be processed at least every 180 days for each patient.
Assist in gathering data for patients whose POT has not been developed timely and creating the late POT for those participants based on information found in the electronic health record, (Medcompass), conversations with the participant and their caregivers, health care providers and fellow staff to be sent to the Health Care provider and participant for signature.
Provide training on submitting the POT to the appropriate HHCM department for processing.
Train nursing and social work staff on completing of specific forms required by the HCBA program. These include but are not limited to: Menu of Health services, Home Safety Evaluation, Acuity Score Form, Monthly Phone call template, Event Issue Reports, Consent for Communication, Excel spreadsheet used to track caseloads, Supplemental orders, Waiver Personal Care Service (WPCS) forms and Notice of Action forms.
Train nurses on performing nursing assessments initially as well as reassessment which are documented and located in MedCompass, frequency based on the acuity or change in condition of the participant.
Train nurses and social workers on navigating the comprehensive Electronic Health Record.
Participate in administrative meetings that involve DHCS, other waiver agencies, service providers, etc. as appropriate.
Participate in management meetings when requested.
Assist in preparing case notes for state audits and state administrative law judge hearings and in conjunction with other staff present findings at hearings.
Perform quality assurance on clinical staff’s caseloads and develop Performance improvement plans with staff who need additional supervision or training.
Communicate with DHCS to clarify policy issues regarding inquiries received from participants and/or clinical staff that need clarification or policy review.
Provide support to staff including case conference about participants and/or applicants; assist with monthly calls if necessary; assist with re-assessment visits if necessary; return calls to applicants/participants; and provide instructions to staff on what to do when working with challenging cases.
Provide support, education and ongoing training to clinical staff.
Facilitate HCBA All Staff Meetings, Refresher Trainings, and Intake Staff Meetings as assigned.
Assist in performing QA review, taking samples records from all HCBA clients managed by clinical staff.
Review and provide ongoing feedback on documents (i.e. Acuity Assessment, Consent for Communication, Protected Health Information, Plan of Treatment, Nursing Assessment, Menu of Health Services) completed by new/ seasoned clinical staff.
Follow up with clinical staff regarding status of POT signatures from participants and health care providers and provide instructions on best practices for follow up.
Train on inter-waiver agency transfers from other waiver agencies.
Train new and existing staff on completing NOA request forms for applicants who no longer want HCBA waiver services, admitted to facility over 30 days, dual enrolled in other programs ie ECM, MediCal hospice, regional center waiver, reduction in services, or unable to contact client, or sign POT.
Train on how to run Medi-Cal eligibility for participants whose Medi-Cal in Medcompass is showing up “ineligible” by the first week of every month.
Provide written documentation and verbal support to HCBA Program Manager and Lead Program Mangers when QA reviews demonstrate improvements are required of staff, or if there is a disciplinary action against clinical staff that involves training issues.
Responsible for initial onboarding training, education and support.
Responsible for consistent check-ins with new staff to provide education and support as needed for the first 90 days, with communication and reporting to the HCBA Program Manager and Lead Program Managers.
Responsible for training, education and staff support as identified by the HCBA Program Manager and/or Lead Program Managers.
Responsible to stay apprised of revisions, changes and additions to all HCBA waiver program regulations and policies and to educate and train HCBA staff.
Responsible for ongoing and consistent training in all HCBA policies and procedures. To include but not limited to office procedures, client processes, forms and reporting etc.
Other duties as assigned by CEO, Director of Operations, and/or HCBA Program Manager.
