About The Position

Sentara Norfolk General Hospital is seeking a full-time, temporary Registered Nurse (RN) Stroke Program Coordinator/Navigator to help drive patient progression, reduce length of stay, and support exceptional outcomes for our stroke population. This role is expected to be in place for approximately eight months, with the potential to become permanent based on measurable success in reducing length of stay (LOS) for stroke patients. In this highly visible role, you will serve as the day-to-day operational lead for stroke patient progression, partnering across disciplines to identify barriers, escalate risks, coordinate care, and support timely, safe transitions throughout the continuum. The Stroke Program Coordinator/ Navigator applies professional nursing judgment, clinical expertise, and technical knowledge to support comprehensive neurological and stroke patient care. This role helps ensure assessment, management, implementation, and evaluation of interventions aligned with accrediting agency requirements, disease-specific program certifications, CMS expectations, and Sentara standards.

Requirements

  • BSN Required
  • Registered Nurse license required
  • Basic Life Support (BLS) required within 90 days of hire
  • 3 years of RN experience Required
  • Stroke Patients-Incumbents are required to be knowledgeable of the standards of care for the stroke patients and meet the annual educational requirements set forth by the System for certified stroke centers.

Responsibilities

  • Daily patient surveillance: Proactively track stroke patients across units to support timely progression of care.
  • Barrier identification and escalation: Identify and escalate delays related to diagnostics, consults, procedures, discharge planning, or clinical changes.
  • Mortality risk mitigation: Recognize high-risk patients early and escalate concerns related to decline, complications, or delays.
  • Care coordination: Align neurology, hospitalists, rehab, case management, nursing, and other key partners around patient needs and next steps.
  • Discharge acceleration: Support early planning for transitions to inpatient rehabilitation, skilled nursing, home health, hospice, or other appropriate settings.
  • Goals-of-care and palliative integration: Prompt early consults for high-risk populations, including SAH and ICH patients as appropriate.
  • Post-discharge continuity: Help ensure patients complete advance care planning prior to discharge when appropriate due to elevated risk for recurrent stroke.
