Requirements

• Active Minnesota Registered Nurse (RN) license.
• Strong clinical judgment and independence.
• Comfortable working in a field-based role.
• Reliable transportation.

Nice To Haves

• Home health experience preferred, but not required.

Responsibilities

• Provide skilled nursing visits in homes and community settings.
• Perform assessments, wound care, and medication management.
• Educate patients and families with clarity and compassion.
• Collaborate with RN Case Managers and the care team.
• Float to nearby territories to support patient needs.
• Maintain accurate, timely documentation.
