About The Position

Atrium Health Transitional Care Clinic provides care for 30 days following a patient's release from the hospital. A dedicated team of doctors, pharmacists, and social workers offer care and support, working with patients to provide resources, tools, and education for a safe, quick, and healthy recovery. Transitional care can be provided in the office, virtually, or at the patient's home, depending on their needs and available resources.

Requirements

• Completion of an accredited or approved program in nursing
• Registered Nurse license issued by the state in which the teammate practices
• Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
• One year of Nursing experience

Responsibilities

• Provide and coordinate comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures.
• Utilize evidence-based practice, clinical decision-making, compassion, and communication skills.
• Lead efforts to create the safest patient environment and the best patient experience across the continuum.
