About The Position

Pacific Hospice and Palliative Care is growing, and we are seeking a compassionate, experienced, and dependable Registered Nurse to join our dedicated hospice team serving patients throughout the San Antonio area. This position is ideal for an RN seeking a PRN, Part-Time, or On-Call opportunity providing in-home hospice care. The ideal candidate must have a minimum of two years of hospice experience, be bilingual in English and Spanish, and reside in the San Antonio area. We are looking for a nurse who combines strong clinical expertise with empathy, professionalism, and excellent communication skills.

Requirements

  • Current and unrestricted Texas RN license
  • Minimum of 2 years of hospice nursing experience required
  • Strong clinical assessment and case management skills
  • Experience with pain management, symptom control, and end-of-life care
  • Proficient with electronic documentation systems, including Axxess or other hospice EMR platforms
  • Comfortable using email, Slack, and other communication platforms
  • Excellent verbal and written communication skills
  • Compassionate, dependable, and team-oriented professional
  • Reliable transportation and valid driver’s license required

Nice To Haves

  • Bilingual English/Spanish preferred
  • Knowledge of Medicare Conditions of Participation and Joint Commission standards preferred

Responsibilities

  • Provide compassionate, patient-centered nursing care to hospice patients in their homes
  • Perform comprehensive assessments, including pain and symptom management, psychosocial support, and ongoing evaluation of patient needs
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team, patients, and families
  • Administer medications and treatments according to physician orders and hospice protocols
  • Educate patients and caregivers regarding disease progression, symptom management, medications, and end-of-life care
  • Provide emotional support and guidance to patients and families throughout the hospice journey
  • Collaborate closely with physicians, social workers, chaplains, aides, and other team members to ensure continuity of care
  • Maintain accurate and timely clinical documentation in compliance with hospice regulations and agency standards
  • Participate in bereavement support and family follow-up as appropriate
  • Stay current with hospice best practices through ongoing education and professional development
