About The Position

The Community Hospice has a wonderful opportunity for a Per diem RN to work as part of our Hospice Care Team caring for patients and families in local Nursing Homes. Needs are on Weekdays (8a-4:30p) as needed. No Weekends available. This role involves case management of patients in the Nursing home setting, responding to symptom management needs of Hospice patients, and coordinating with the team to provide support to residents and families. You will also work with an interdisciplinary team.

Requirements

• Valid NYS RN license required
• Minimum 1 year RN experience required
• Clean Driver's License and reliable vehicle
• Strong interpersonal skills to establish and maintain relationships with patients, families, physicians and coworkers

Nice To Haves

• 1-2 years long term care RN experience preferred
• Previous Hospice experience preferred
• Ability to learn/use an electronic medical record

Responsibilities

• Case manage patients in the Nursing home setting
• Respond to symptom management needs of Hospice patients
• Coordinate with the team to provide added support to residents and families
