About The Position

AdvisaCare is seeking a compassionate and dynamic Per Diem Hospice Registered Nurse (RN) to join their growing Hospice Family in Flint, servicing Port Huron and surrounding areas. This role focuses on providing one-on-one care in the comfort of patients' homes, emphasizing presence, compassion, and honoring each moment. The Hospice RN will support patients and families with dignity and heart, aiming to help patients live their final days with dignity and comfort. Responsibilities include conducting supervisory visits for clinical observation of assigned team members, participating in interdisciplinary group meetings, and contributing to the development, review, and revision of the patient/family plan of care. The position requires on-call responsibilities.

Requirements

Current Michigan License in good standing
Minimum 1 year hospice experience
Empathetic approach to patients and caregivers

Responsibilities

Provide one-on-one care in the comfort of home
Support patients and families with dignity and heart
Spend real time where it matters most
Provide emotional support to help patients live their final days with dignity and comfort
Conduct appropriate supervisory visits for clinical observation of performance of assigned team members
Participate and provide feedback to the team for regular interdisciplinary group meetings
Contribute to the development, review, and revision of the patient/family plan of care
Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs
Implement the individualized plan of care and revisions as needed
Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
Supervision of paraprofessionals providing services to patient
Preparing clinical and progress notes
Participating in IDG conferences
Participate in Hospice performance improvement program
Maintain high level of customer satisfaction with our clients, families and caregivers
Communication of information using current process and technology within organization
Participate in field visits daily
Participate in weekly office meetings as indicated
Ability to adapt to patient’s status and needs on a full time on call basis
