This position utilizes the nursing process to assess patient needs, plan and implement individualized interventions, and evaluate the effectiveness of the care plan. It involves the patient and family in care planning and implementation, fostering a therapeutic relationship throughout the healthcare continuum. The role requires anticipating and recognizing significant changes in patient condition, status, and care needs, providing ongoing assessment, and taking appropriate action when changes occur. The nurse will anticipate and alter the plan of care as needed, mobilize appropriate resources, and collaborate with other healthcare team members to coordinate patient-centered care. Strategies to reduce patient risk and increase patient safety will be implemented. The role also involves assessing patient and family readiness to learn, individualizing the learning approach, and developing strategies to meet learning needs. Active coordination of the discharge plan using an interdisciplinary approach is expected. The nurse will apply evidence-based practice, incorporate nursing and regulatory standards of care, and provide ongoing evaluation of patient, team, and unit outcomes, taking appropriate action when changes are needed. Support for shared governance activities and initiatives to improve processes and patient outcomes is also part of the role. Effective communication, feedback, and conflict resolution skills are essential, as is promoting collaboration among healthcare team members. The nurse will assume a leadership role in the care team and delegate appropriately to unlicensed personnel. Promoting a culture of safety by identifying threats to patient safety and intervening to prevent harm is crucial. Reporting patient safety events and near misses in a timely manner, seeking to identify potential safety issues, and assisting in the implementation of corrective actions are also key responsibilities. Ethical decision-making, respect for peers and other clinical disciplines, and participation as an effective member of the patient care team to formulate an integrated, unbiased, individualized approach to care are expected. Participation in quality/process improvement initiatives and professional activities that contribute to personal and others' development is encouraged. Seeking opportunities to be taught, coached, and mentored, and attending required meetings/educational programs and completing annual competencies in a timely manner are also required. The role requires demonstrating knowledge and skills necessary to provide care appropriate to the age of the patients served, understanding the principles of growth and development over the life span, and possessing the ability to assess and interpret data reflective of the patient's status and needs.
