This role involves planning, organizing, directing, and delivering nursing services in collaboration with the interdisciplinary team. The RN On-Call will develop individualized care plans, address urgent patient needs, and assist families in maintaining comfort and coping during after-hours emergencies. HOURS / SCHEDULE : Full-time, 7 days on / 7 days off (on-call role), Monday through Monday Monday–Friday: On call from 5:00 PM to 8:00 AM. Respond to calls and complete visits as needed. Weekend Coverage (Saturday–Monday): On call continuously through Monday at 8:00 AM, including answering calls and completing visits as needed. Triage all after-hours needs during this period.

Requirements

  • Associate degree in nursing from an accredited college or university (or international equivalent) preferred.
  • Minimum of two (2) years of hospice nursing experience required.
  • Current RN license in the assigned state.
  • Current CPR certification is required.
  • Valid driver’s license with a reliable, insured vehicle that meets organizational requirements.
  • Ability to write, speak, understand, and read the English language.
  • Demonstrate knowledge of and supports Affinity Hospice’s vision, value statements, standards, policies, and procedures.
  • Excellent interpersonal, communication, and time management skills.
  • High organizational and work ethic standards.
  • Strong analytical and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Must be computer literate.
  • Flexibility to travel within the assigned territory.
  • Meet mandatory continuing education requirements.
  • Demonstrate commitment, professional growth, and competency.

Responsibilities

  • Ability to work on call night and weekends as needed.
  • Work closely with the sales, clinical, and leadership team.
  • Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
  • Implement/develop/document the patient plan of care.
  • Provide care utilizing infection control measures that protect both the staff and the patient (OSHA).
  • Assure continuity of quality patient care delivered with appropriate documentation.
  • Monitor assigned cases to ensure compliance with requirements of third-party payer.
  • Promote agency philosophy and administrative policies.
  • Perform on-call responsibilities and provide on-call service to patients/families as assigned.
  • Supervise and provide directions to the home health aide and the LPN.
  • Perform other duties as assigned by management to support department goals and initiatives.
