About The Position

We are seeking a compassionate and dependable Registered Nurse (RN) to provide high-quality, resident-centered nursing care in a skilled nursing and long-term care environment. The RN will assess residents, develop and coordinate individualized care plans, administer medications and treatments, monitor changes in condition, communicate with physicians and families, and ensure residents receive safe, respectful, and efficient care.

Requirements

Graduate of an accredited school of nursing.
Current Registered Nurse (RN) license in the applicable state of practice.
BLS certification required.
Knowledge of basic nursing practices and procedures.
Knowledge of medications, including therapeutic effects and potential side effects.
Strong observation, assessment, and communication skills.
Ability to accurately measure and interpret relevant clinical information.
Ability to effectively communicate in English, both verbally and in writing.
Ability to use an Electronic Medical Record (EMR) for clinical documentation.
Frequently stand, walk, move, and reposition oneself while performing nursing duties.
Communicate effectively with residents, families, physicians, and coworkers.
Observe and assess residents and respond appropriately to changes in condition.
Operate and use nursing equipment and controls.
Assist with resident transfers and repositioning using appropriate equipment and techniques.
Lift and/or move up to 35 lbs.
Maintain appropriate body positioning while providing resident care.

Previous long-term care or skilled nursing experience is beneficial.

Responsibilities

Assess residents' physical, emotional, and psychosocial needs.
Develop, implement, and coordinate individualized plans of care.
Evaluate resident progress and update care plans as needed.
Administer medications and treatments according to physician orders and established policies.
Monitor residents for changes in condition and promptly report concerns to appropriate healthcare providers.
Ensure physician orders are processed accurately and in a timely manner.
Maintain accurate and complete documentation in the Electronic Medical Record (EMR).
Safeguard residents' personal property and confidential health information.
Protect residents' privacy, dignity, and self-esteem.
Promote resident independence and provide opportunities for choice consistent with individualized care plans.
Respond promptly to resident call lights and requests.
Maintain safe, clean, pleasant, and orderly resident living areas.
Provide education and support to residents and their families/significant others.
Receive, communicate, and document important resident information during shift-to-shift care.
Direct and coordinate care provided by other members of the nursing team as appropriate.
Use gait belts, mechanical lifts, slide boards, and other transfer equipment safely and correctly.
Apply appropriate resident-handling techniques to promote safety and comfort.
Follow department-specific emergency, fire, disaster, and safety procedures.
Maintain compliance with applicable laws, regulations, policies, and professional nursing standards.
Participate in required training, meetings, and continuing education.
Perform other duties as assigned.
