About The Position

We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality, resident-centered care in a long-term care and short-stay rehabilitation setting. The RN will assess residents, develop and coordinate individualized care plans, administer medications and treatments, monitor changes in condition, provide wound care, and collaborate with residents, families, physicians, and interdisciplinary team members to promote positive outcomes.

Requirements

  • Graduate of an accredited nursing program.
  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).
  • Current Registered Nurse (RN) license in the applicable state of practice.
  • BLS certification required.
  • Knowledge of basic nursing practices and medication administration.
  • Ability to effectively communicate and comprehend information in English.
  • Ability to accurately document care using an EMR.

Responsibilities

  • Perform comprehensive nursing assessments and identify resident care needs.
  • Develop, implement, and coordinate individualized plans of care.
  • Evaluate residents' progress and modify care plans as appropriate.
  • Administer medications and treatments safely and accurately.
  • Provide and monitor wound care according to established protocols and individualized care plans.
  • Observe residents for changes in physical, emotional, and behavioral condition.
  • Promptly report significant changes in resident condition to physicians and appropriate healthcare team members.
  • Ensure physician orders are processed accurately and in a timely manner.
  • Maintain accurate and timely documentation in the Electronic Medical Record (EMR).
  • Monitor and document resident responses to treatments and interventions.
  • Respond promptly to resident call lights and requests.
  • Promote resident dignity, privacy, independence, and choice.
  • Safeguard resident property and confidential health information.
  • Provide education and support to residents and their families/significant others.
  • Coordinate care with nursing assistants, rehabilitation professionals, physicians, and other interdisciplinary team members.
  • Safely use gait belts, mechanical lifts, slide boards, and other transfer equipment.
  • Apply appropriate resident-handling techniques to promote safety and comfort.
  • Maintain a safe, clean, and orderly resident environment.
  • Follow infection prevention, safety, emergency, fire, and disaster procedures.
  • Participate in quality improvement initiatives, staff meetings, training, and educational programs.
  • Perform other duties as assigned.
