About The Position
Eddy Visiting Nurse and Rehab Association is seeking a full-time Registered Nurse (RN) Liaison to join their team at St. Peter's Hospital. This role involves assisting with homecare referrals and discharge planning, working closely with community providers to secure necessary documents, and participating in patient rounds to assess those preparing to return home. The ideal candidate will possess strong attention to detail, excellent communication skills, and organizational abilities, along with a solid understanding of homecare regulations.
Requirements
- NYS Registered Nurse license
- 1 year of RN experience
- Valid NYS driver's license
- Strong interpersonal skills
- Excellent computer skills
- Ability to multitask multiple patient referrals
- Organization
- Team player
- Strong time management and clinical skills
- Understanding of homecare regulations
Nice To Haves
- Experience working with seniors and/or in HomeCare preferred
- Experience in discharge planning and case managing a plus!
