About The Position

The VNA of Manchester and Southern New Hampshire provides 24/7 in-home support and services to patients in Manchester and the surrounding communities. We are committed to helping our patients navigate the challenges they face at home while recovering from surgery or coping with life-limiting illnesses or physical disabilities. The VNA care team provides individualized care across the continuum, bringing peace of mind to patients and their loved ones while providing professional healthcare services in the comfort of home.

Requirements

  • Bachelor's degree in sales and marketing or equivalent experience required
  • A minimum of two (2) years in sales and marketing or Healthcare.
  • Valid Driver's license required
  • Familiar with MS Outlook, Work, Excel and/or Access.
  • Familiar with setting up and maintaining a data base of contacts and activities.
  • Ability to execute sales plan, use professional selling skills and have territory management strategies necessary to build the service lines of the Elliot VNA.
  • Strong analytical, strategic, and creative problem-solving skills.

Nice To Haves

  • Two (2) years of experience in sales and marketing with a proven track record preferred.
  • Experience in preparing and providing education programs preferred.
  • Familiar with Epic, MS Outlook, Work, Excel preferred.
  • Familiar with setting up and maintaining a data base of contacts and activities preferred.

Responsibilities

  • Build, establish and maintain strong referral relationships with Skilled Nursing Facilities (SNF), nursing facilities, physicians and physician practices, hospital and discharge case managers, assisted living facilities and the community ensuring all patients who would require and benefit from VNA services have access to those services.
  • Coordinate transitions to the agency.
  • Provide in-services to facilities related to home health and hospice care.
  • Promote and educate about all VNA programs and services.
  • Assist with access and transitions of care to homecare and hospice by building long-term relationships and ensuring excellent customer service.
  • Ensure the agency is a preferred provider.
  • Facilitate cooperation between and among Facility and Hospice staff.
  • Explain homecare and hospice services to potential patients and family members.
  • Determine appropriateness of homecare referrals; gather pertinent clinical information and coordinate and conduct preadmission activities for homecare.
  • Determine appropriateness of home health and hospice referrals.
  • Make regular site visits to facilitate referral coordination.
  • Effectively transition patients to the VNA.
  • Work with Director to develop and implement a plan to enhance all VNA services and grow referral volume.
  • Identify new services to address the needs of customers and provide education to referral sources regarding services.
