About The Position

Suncrest Hospice aims to redefine hospice care by offering exceptional service and care to patients. This is achieved by increasing staff to patient ratios, which allows for more clinical visits and lower clinician caseloads. We are seeking individuals committed to providing the highest quality of care to patients and their families.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
• Ability to work collaboratively within a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to work independently on day-to-day activities, including phone, email, and written communication.
• Ability to attend meetings with various healthcare professionals.
• Understanding of hospice rules and regulations.
• Time management skills to manage visits, documentation, and meetings.
• Ability to prioritize and triage multiple concerns.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, a valid driver's license, and auto insurance to travel across the service area.

Responsibilities

• Provide care and services for a group of patients.
• Visit patients in their residences, including private homes, Skilled Nursing Facilities, and assisted living facilities.
• Serve as a contributing member of the interdisciplinary team.
• Provide expertise in psych-social assessments, interventions, and knowledge of community resources.
• Work in a Case Management, Admissions, or Resource role.
