Registered Nurse (RN), Hospice Admissions
About The Position
Phoenix Hospice is hiring a Hospice Admissions Registered Nurse to guide patients and families as they transition from curative care to comfort-focused hospice care. The role combines clinical judgment with compassionate support, addressing patients’ physical needs as well as emotional, spiritual, and psychosocial needs. The nurse collaborates with intake coordinators, hospice consultants, medical directors, facility staff, physicians, and the interdisciplinary hospice team to ensure admissions are smooth and centered on patients’ needs. The position serves Springfield, Missouri, and surrounding areas. It is full time, with 30–40 hours per week, 10-hour shifts, Saturday through Monday.
Requirements
- Hospice experience is required.
- Active Missouri or Compact RN license.
- Valid driver’s license.
- Strong assessment, organization, and communication skills.
- Ability to work independently and as part of a team.
Responsibilities
- Complete hospice admissions and skilled nursing visits.
- Provide compassionate care and support to patients and families.
- Educate patients and families about conditions such as CHF, COPD, and diabetes.
- Provide wound care and medication management.
- Document patient conditions and communicate changes to physicians.
- Collaborate with the interdisciplinary hospice team and relevant care partners to support smooth admissions.
