They are seeking a compassionate, organized, and team-oriented individual to join their Home Health team as a Full Time or Part Time (Per Diem) Registered Nurse (RN) in Brentwood, Antioch, Oakley, Discovery Bay. Requirements

Current Registered Nurse (RN) License to practice in California - Required
Experience in Home Health or Hospice care - 1 Year Required
Current CA Driver’s License
Able to travel within a 15 - 25 mile radius on a regular basis.

Responsibilities

Provide nursing care within the scope of practice, following the patient’s individualized care plan, in coordination with the Interdisciplinary Team.
Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
Supervise Licensed Vocational Nurses (LVNs) when assigned.
Comply with all company policies and procedures to ensure high standards of care.
