Registered Nurse (RN) Home Health - Part Time (Per Diem)

We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full Time or Part Time (Per Diem) Registered Nurse (RN) in Brentwood, Antioch, Oakley, Discovery Bay. Requirements
- Current Registered Nurse (RN) License to practice in California - Required
- Experience in Home Health or Hospice care - 1 Year Required
- Current CA Driver’s License
- Able to travel within a 15 - 25 mile radius on a regular basis.
Responsibilities
- Provide nursing care within the scope of practice, following the patient’s individualized care plan, in coordination with the Interdisciplinary Team.
- Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
- Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
- Supervise Licensed Vocational Nurses (LVNs) when assigned.
- Comply with all company policies and procedures to ensure high standards of care.
