Registered Nurse (RN) - Home Health - Part-time
About The Position
Comfort Home Care is seeking a part-time Registered Nurse (RN) for home health in the Salem/Peabody area, working an afternoon weekday shift. This role offers a better work-life balance and the opportunity to build meaningful relationships with patients, providing compassionate care in their homes. The position allows for flexibility, autonomy, and support, enabling nurses to thrive both professionally and personally. Nurses help psychiatric and home health patients maintain independence, prevent hospitalizations, and receive care at home. The role involves assessing patients, creating and modifying care plans, teaching and supporting patients, communicating with care teams, and documenting visits from home.
Requirements
- Active Registered Nurse (RN) license in Massachusetts.
- One year of nursing experience (home health or psych preferred).
- A drive to provide compassionate, patient-centered care.
- Reliable transportation for patient visits.
Responsibilities
- Assess patient's current condition, medical needs, and overall well-being during each visit.
- Create and modify patient care plans based on their needs and changing conditions.
- Teach and support patients, including medication guidance for psychiatric patients and wound care for post-op patients.
- Communicate and coordinate with providers, families, and care teams to ensure patients receive appropriate care.
- Document patient visit notes accurately from home for compliance and continuity of care.
