The Home Health and Hospice RN is responsible for client care, including direct nursing care, coordination of care, and coordination of services in the home for clients of all ages to assist them in achieving a maximum level of wellness and independence.

Requirements

  • Diploma, Associates or Bachelor's degree from a college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
  • Current Registered Nurse license in the State of Iowa.
  • Must obtain CPR certification within 3 months of hire date.
  • Access to automobile required.
  • Valid driver’s license.
  • Must carry their own automobile liability insurance.

Nice To Haves

  • Bi-lingual experience preferred but not required.

Responsibilities

  • Coordinating client services, case management, providing assigned clients with direct skilled care, and teaching patient care skills to the client and/or caregiver.
  • Initiating preventative and rehabilitative nursing procedures individualized to the client and according to age specific needs.
  • Developing and revising a plan of care based on the initial assessment, continual reassessment of client/family needs, client history, nursing diagnosis, physician orders, expected outcomes, and client goals to include discharge plans when appropriate.
  • Utilizing appropriate agency and community resources, acting as a client/family advocate, and following visit utilization guidelines and protocols.
  • Formulating and implementing appropriate teaching plans.
  • Communicating significant clinical findings and changes in status to the physician and other involved personnel, including the clients.
  • Thoroughly documenting all skilled and medically necessary care timely and accurately, ensuring compliance with Medicare, State, Federal, insurance, and payer requirements, as well as local agency policy.
  • Maintaining client and organizational confidentiality at all times.
  • Actively participating in the QAPI (Quality Assurance and Performance Improvement) program and completing all required in-service programs.
  • Conducting Home Health Aide supervisory visits, preparing and updating Home Health Aide’s written instructions, and documenting findings.
  • Ensuring clients receive appropriate care and all duties and activities delegated to aides are within the scope of the Nurse Practice Act.
  • Collecting patient health data.
  • Documenting findings utilizing Meditech Home Care and completing appropriate OASIS time point completely and accurately.
  • Identifying expected outcomes/goals individualized to the patient and developing a plan of care that prescribes interventions to attain expected outcomes.
  • Collaborating with the patient, significant others, and health care providers in providing patient care.
  • Attending and preparing for active participation in every other week care conferences.
  • Contributing to the professional development of peers, colleagues, and others.
  • Making decisions and actions on behalf of clients in an ethical manner.
  • Considering factors related to safety, effectiveness, and cost in planning and delivering patient care.
  • Assisting with the intake of referrals in Home care, assuring Medicare admission criteria are met, and obtaining benefits and preauthorization of services for commercial payers as indicated.
  • Performing other duties and responsibilities as assigned.
