We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full Time or Part Time (Per Diem) Registered Nurse (RN) in San Francisco, Daly City, Millbrae, Brisbane. Requirements

• Current Registered Nurse (RN) License to practice in California - Required
• Experience in Home Health or Hospice care - 1 Year Required
• Current CA Driver’s License
• Able to travel within a 15 - 25 mile radius on a regular basis.

Responsibilities

• Provide nursing care within the scope of practice, following the patient’s individualized care plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Supervise Licensed Vocational Nurses (LVNs) when assigned.
• Comply with all company policies and procedures to ensure high standards of care.
