Registered Nurse (RN) - Community Hospice - Nursing Homes
About The Position
Hospice Registered Nurse (RN) for Nursing Homes in Columbia & Greene Counties. This full-time position offers the rewards of working in end-of-life care with no weekends or on-call duties. The Community Hospice has an opportunity for an RN to join our Hospice Care Team, caring for patients and families in local Nursing Homes. Work days are Monday to Friday, 8 am to 4:30 pm. Case managers are assigned specific nursing homes to minimize travel. Benefits include national pre-taxed mileage reimbursement, meetings and charting conducted via supplied laptop, comprehensive orientation, and full benefits through St Peters Health partners.
Requirements
- A current license to practice as a Registered Nurse in the State of New York.
- Associates Degree in Nursing, BSN preferred.
- 1 year experience as an RN required.
- Long term care experience is a MUST.
- Valid Driver's License and Reliable vehicle.
- The RN must be able to communicate effectively, verbally and written.
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers.
Nice To Haves
- Previous hospice experience preferred but NOT required.
Responsibilities
- Case manage patients in the Nursing home setting.
- Respond to symptom management needs of Hospice patients.
- Coordinate with the team to provide added support to residents and families.
- Collaborate with the nursing home in the care of the hospice patient.
