The Community Hospice has an exciting opportunity for a Registered Nurse (RN) to join our dedicated Hospice Care Team, serving patients and families throughout the Capital Region. Shift Details: Saturday & Sunday, 8:00 AM – 8:00 PM once trained. Will require some weekday training in the beginning. Eligible for Full-Time Medical Benefits.
Requirements
- Valid NYS RN license
- Associate’s degree in nursing
- Minimum 1 year RN experience required
- Valid driver’s license and reliable vehicle.
- Willingness to go to multiple counties as needed
- Ability to navigate through EMR (electronic medical records) platforms
- Good communication skills.
Nice To Haves
- 1–2 years of home care or hospice experience preferred
Responsibilities
- Visit patients in their home or facility to respond to symptom management needs and coordinate with our team to provide end of life support to patients and families.
- Work closely with an interdisciplinary team that includes RNs, LPNs, aides, physicians, social workers, chaplains, complementary therapists, and volunteers.
