About The Position

The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as a Liaison/admission RN/hospice RN seeing patients in local hospitals (Capital Region- primarily Albany Medical Center, St. Peter's Hospital). This role is eligible for Full-Time Medical Benefits. The schedule is only every Saturday and Sunday from 7 am to 7:30 pm, with no days during the week after the training period. Requirements

A current license to practice as a Registered Nurse in the State of New York
Associates Degree in Nursing, BSN preferred
Minimum 1 year recent acute care RN experience
Valid Driver's License and Reliable vehicle
High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers

Nice To Haves

Hospice or RN Liaison experience a plus but not required.
Understanding of Hospice principles preferred.

Responsibilities

Evaluate patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care.
Coordinates with the hospital providers and our hospice medical directors to confirm eligibility for hospice services.
Gather information and ensure a smooth handoff to the hospice RN admitting the patient/family to services.
Admits patients to hospice inpatient services upon eligibility.
Develops a hospice plan of care and collaborates with the hospice interdisciplinary team and the acute care team.
Work closely with the hospital case management department.
