About The Position

Full-Time Hospice Registered Nurse (RN) Admissions - Community Hospice. Seeking a candidate to work 4- 10 hour days/evenings (10 am-8 pm). Start your day from home once trained! The Community Hospice has a rewarding opportunity for a full-time RN to work as a part of our Admissions Team in Hospice Home Care with cross training for admissions to other levels of care. The Hospice Admissions RN admits patients to home care hospice services. The workday includes two home care admissions to Hospice. Will go to patients' homes to complete the initial assessment of patient/family needs and begins implementation including ordering medications, medical equipment, supplies and support from our interdisciplinary team which includes social workers, chaplains, volunteers, complementary therapists, home health aides, and more! Ensures a smooth hand-off to the Hospice Case Manager. Corporate office is 445 New Karner Road-Albany NY with satellite offices in Catskill and Saratoga NY. Admissions are across 8 counties: Columbia, Greene, Albany, Rensselaer, Schenectady, Saratoga, Washington. Your region is considered in patient assignment.

Requirements

• A current license to practice as a Registered Nurse in the State of New York
• Associates Degree in Nursing, BSN preferred
• 1 year experience as an RN required
• Ability to navigate through several EMR platforms
• Good communication skills to collaborate with community partners and work with an interdisciplinary team.
• Reliable Vehicle and Driver's License

Nice To Haves

• 2 years RN experience with recent acute and/or hospice experience is preferred
• BSN preferred

Responsibilities

• Coordinate with the patient's community physician and our hospice medical directors to confirm eligibility for hospice services.
• Complete the initial assessment of patient/family needs and begins implementation including ordering medications, medical equipment, supplies and support from our interdisciplinary team.
• Ensure a smooth hand-off to the Hospice Case Manager.
