About The Position

This Full-Time medical benefits eligible weekend track position is perfect for an RN with weekday responsibilities but open to work every weekend at an enhanced rate. The Community Hospice has a wonderful opportunity for a FT RN to work as part of our Hospice Care Team caring for patients and families in the homecare and nursing home setting on the weekends in the Capital District and Saratoga areas. Community Hospice provides a thorough orientation, specialized training in end-of-life care, and a supportive work environment where your skills and compassion truly make a difference. From comprehensive training to providing necessary protective equipment, we're committed to creating a secure environment for you as you deliver exceptional care in our clients' homes. This role offers the chance to make a meaningful impact while working closely with an interdisciplinary team that includes RNs, LPNs, aides, physicians, social workers, chaplains, complementary therapists, and volunteers.

Requirements

Valid NYS RN license
Associate’s degree in nursing
Minimum 1 year RN experience required
Valid driver’s license and reliable vehicle.
Willingness to go to multiple counties as needed
Ability to navigate through EMR (electronic medical records) platforms
Good communication skills.

Nice To Haves

1–2 years of home care or hospice experience preferred

Responsibilities

Visit patients in their home or facility to respond to symptom management needs and coordinate with our team to provide end of life support to patients and families.
