The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as a Liaison and Hospice RN in local hospitals (Capital Region-primarily Ellis and Samaritan Hospitals). Schedule: Only Every Saturday and Sunday 7am - 7:30pm. Coordinate with the patient’s community physician and our hospice medical directors to confirm eligibility for hospice services. Gather information and ensure a smooth handoff to the hospice RN admitting the patient/family to services. Admits patients to hospice inpatient services upon eligibility, Develops a hospice plan of care and collaborates with the hospice interdisciplinary team and the acute care team. Work closely with the hospital case management department. What you will need: A current license to practice as a Registered Nurse in the State of New York Associates Degree in Nursing, BSN preferred 1-2 years recent acute care RN experience Hospice or RN Liaison experience a plus but not required. The RN must be able to communicate effectively, verbally and written Valid Driver's License and Reliable vehicle High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers Understanding of Hospice principles preferred Training provided in end of life care. Enhanced rate for weekend track: $57.90/hr Experience based pay range for NON-weekend track: $38.00-$50.18 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
