Requirements

• Active North Carolina Registered Nurse license
• Current CPR/BLS certification
• Excellent communication and relationship-building skills
• Organized, dependable, and self-motivated
• Reliable transportation with willingness to travel throughout an assigned territory

Nice To Haves

• Home care, geriatrics, hospice, case management, or community-based nursing experience

Responsibilities

• Conduct in-home nursing assessments and develop individualized service plans
• Complete supervisory visits to ensure quality care and regulatory compliance
• Train, coach, and support caregivers in delivering exceptional care
• Monitor client well-being and communicate changes appropriately
• Educate clients and families while serving as a trusted clinical resource
• Maintain accurate documentation and clinical records
• Collaborate with leadership to improve quality, caregiver performance, and client outcomes
