About The Position

The Clinical Care Supervisor is a Registered Nurse role combining case management and leadership responsibilities with direct skilled nursing care for pediatric home health patients. The supervisor provides clinical mentorship, precepting, and liaison support while delivering hands-on care in patients’ homes.

The role involves working a combination of field days and on-site office days. In the field, the supervisor provides nursing care and completes home visits across the service area. In the office, the supervisor supports clinical operations, staff training, and administrative duties.

Requirements

• Registered Nurse (RN) license required.
• Graduate of an accredited school of nursing.
• Pediatric competency and strong clinical judgment.
• Ability to work independently.
• Routine travel within the service area is required.

Responsibilities

• Deliver skilled nursing services according to patient care plans, agency policies, and physician orders.
• Demonstrate pediatric competency and sound clinical judgment while working independently.
• Assist with managing a caseload of clients, developing personalized treatment plans, and coordinating care.
• Assist with onboarding new nursing hires, including orientation and competency check-offs.
• Mentor and precept direct care staff in home and office settings.
• Conduct comprehensive assessments, supervisory visits, and re-evaluations to support quality patient care.
• Provide direct nursing care during shifts in patients’ homes.
• Support clinical operations, staff training, and administrative duties while working on-site at the office.
• Travel routinely within the service area for patient home visits.
