About The Position

This role provides a different experience than hospital, clinic, or TCU settings, focusing on one-on-one patient care in homes and community settings. The position offers autonomy over caseload and daily schedule, with a consistent weekday schedule and a manageable on-call rotation. Requirements
- Active Minnesota Registered Nurse (RN) license
- 3+ years of RN experience, home health preferred
- Strong assessment, care planning, and documentation skills
- Ability to work independently in a field-based role
- Reliable transportation and valid driver’s license
Responsibilities
- Complete comprehensive nursing assessments and care plans
- Perform OASIS visits and coordinate care across settings
- Provide skilled nursing care, education, and support to patients and families
- Collaborate with physicians, therapists, and interdisciplinary teams
- Manage a caseload of patients in homes, assisted living, and community settings
- Maintain accurate, timely documentation and regulatory compliance
