About The Position

This role is for a Registered Nurse (RN) Case Manager in Home Health, offering clinical autonomy and leadership. Requirements

• Active Minnesota Registered Nurse (RN) license.
• 3+ years of RN experience, home health preferred.

• Confidence working independently in a field-based role.
• Organized, accountable, and patient-focused.
• Reliable transportation.
