About The Position

Hospital and clinic roles often mean high ratios, constant interruptions, and limited time with patients. Home health is different. You provide 1:1 patient care, manage your own day, and actually see the impact of your work over time. You are not rushing room to room. You are thinking, assessing, educating, and connecting. Requirements

• Active Minnesota Registered Nurse (RN) license.
• 3+ years of RN experience, home health preferred.
• Strong clinical judgment, assessment, and documentation skills.
• Ability to work independently in a field-based role.
• Reliable transportation.

Responsibilities

• Manage a home health caseload across homes and community settings.
• Complete OASIS assessments and individualized care plans.
• Provide skilled nursing care, education, and care coordination.
• Work closely with physicians, therapists, and interdisciplinary teams.
• Support patients in improving function, safety, and independence.
• Maintain accurate, timely documentation.
