About The Position

Professional Case Management is hiring Registered Nurse Case Managers in Tennessee to provide in-home care to former Nuclear Weapons Workers living with chronic or terminal illnesses resulting from toxic-substance exposure during their work for the Government. Case Managers conduct weekly nursing visits, typically lasting one to two hours. Visits include physical assessments and vital signs, checking home safety with medical equipment and supplies, filling weekly medication planners, and overseeing skilled and non-skilled caregivers in the client’s home. The role offers one-on-one patient relationships and the opportunity to support clients’ health, quality of life, and peace of mind. Nurses create their own schedules, with flexibility to pursue school, travel, volunteer work, or personal activities. The Case Manager provides direct care and case management for assigned clients; plans, implements, and evaluates care; and participates in, coordinates, and manages client care conferences as needed. The role collaborates with regional and other personnel to address issues and improve services, supports staffing decisions within scope of practice and client needs, and serves as a local clinical resource. Additional duties may include supporting personnel selection, training and competency validation; reviewing clinical documentation and nurse shift reports; conducting supervisory visits; tracking equipment and staffing; collecting performance and quality data; and assisting with marketing activities. The senior case manager or a similarly qualified alternate must be available during operating hours and participate in relevant professional-service activities, including personnel qualification development and assignment. The listed standard rate is $42 per hour, with shift differentials where applicable. Weekly pay includes 100% of the hourly wage for drive time. Benefits are available.
