About The Position

Empyrean Hospice hires Registered Nurse Case Managers to plan, organize, and direct hospice care with emphasis on community health education and experience. The salary or hourly rate listed is the base for this role; increase commensurate with experience. This role involves representing Empyrean Hospice with the utmost professionalism and compassion, completing comprehensive patient and family assessments to determine hospice needs, and initiating and developing individual care plans with patient and family involvement. The nurse will coordinate patient care for their caseload, counsel patients and families, assist with daily living activities, and maintain accurate clinical notes. Communication with physicians and the hospice care team regarding patient needs and changes in condition is crucial, as is providing and maintaining a safe environment for the patient. The role also includes supervising ancillary personnel and ensuring ethical business and clinical practice standards are prioritized.

Requirements

  • Professionalism, compassion, empathy, and a desire to help others
  • Self-directing with the ability to work with little direct supervision, yet also as member of a complete interdisciplinary care team
  • Understand the philosophy of hospice care
  • Current licensure in State and CPR certification
  • Graduate of an accredited school of nursing with 1-2 years of recent acute care experience in an institutional setting, and 1-2 years of recent experience in home care
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
  • Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

Nice To Haves

  • Bachelor’s degree, with one year of hospice care experience preferred
  • Management experience not required, but will be responsible for supervising hospice aides
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
  • Able to lift, position and/or transfer patients, and able to lift supplies and equipment

Responsibilities

  • Complete an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs. Provide a complete physical assessment and history of current and previous illness(es)
  • Initiate and develop an individual care plan that establishes goals that includes the patient and the family in the planning process
  • Assume responsibility to coordinate patient care for assigned caseload
  • Counsel the patient and family in meeting nursing and related needs and provide health care instructions to the patient as appropriate
  • Assist the patient with the activities of daily living and facilitate the patient’s efforts toward self-sufficiency and optional comfort care
  • Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given
  • Communicate with the physician and hospice care team regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physician orders as required; and work to provide continuity of care
  • Provide and maintain a safe environment for the patient and arrange equipment and other necessary items and services as needed
  • Supervise ancillary personnel and delegates responsibilities when required
  • Ensure standards of ethical business and clinical practice are prioritized
