About The Position

Reserve Care (formerly Hospice of the Western Reserve) is seeking a Registered Nurse (RN) Care Navigation Team Leader to supervise the Care Navigation team. This role supports the implementation and growth of the Care Navigation Program, ensuring high-quality patient and referral source experiences. Requirements

Registered Nurse (RN) in the state of Ohio strongly preferred
Minimum of two years’ hospice experience required
Strong communication, leadership, and managerial skills
Ability to lead a large, multidisciplinary team, including managing performance and disciplinary action when necessary
Strong technology proficiency
Proven ability to manage multiple priorities and operational demands
Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
Provide an active driver’s license
Ability to provide proof of eligibility to work in the United States.
High level of integrity, ethics and professionalism.

Responsibilities

Supervise Care Navigation staff including: Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Care Navigation Social Workers, Care Navigation Support Agents.
Carry out additional supervisory assignments as directed by the Director of Navigation Services.
Participate in the planning and implementation of the Care Navigation Team Development Project.
Assist with onboarding and training of new Care Navigation staff.
Provide supervisory support for evening triage as needed.
Participate in quality, performance, and process improvement initiatives.
Cross-train to provide coverage for additional Care Solutions Center leadership roles including: On-Call Teams, Admissions/Liaison, Triage.
