About The Position

Reserve Care (formerly Hospice of the Western Reserve) is seeking a Registered Nurse (RN) Care Navigation Team Leader to supervise the Care Navigation team. This role supports the implementation and growth of the Care Navigation Program, ensuring high-quality patient and referral source experiences. Requirements

  • Registered Nurse (RN) in the state of Ohio strongly preferred
  • Minimum of two years’ hospice experience required
  • Strong communication, leadership, and managerial skills
  • Ability to lead a large, multidisciplinary team, including managing performance and disciplinary action when necessary
  • Strong technology proficiency
  • Proven ability to manage multiple priorities and operational demands
  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
  • Provide an active driver’s license
  • Ability to provide proof of eligibility to work in the United States.
  • High level of integrity, ethics and professionalism.

Responsibilities

  • Supervise Care Navigation staff including: Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Care Navigation Social Workers, Care Navigation Support Agents.
  • Carry out additional supervisory assignments as directed by the Director of Navigation Services.
  • Participate in the planning and implementation of the Care Navigation Team Development Project.
  • Assist with onboarding and training of new Care Navigation staff.
  • Provide supervisory support for evening triage as needed.
  • Participate in quality, performance, and process improvement initiatives.
  • Cross-train to provide coverage for additional Care Solutions Center leadership roles including: On-Call Teams, Admissions/Liaison, Triage.
