About The Position

Per Diem Hospice Registered Nurse (RN) Intake/Admissions - Community Hospice. This role is for an RN who values meaningful connections and wants to make a difference at one of life’s most important moments. Join The Community Hospice as part of our Central Intake & Admissions Team, where your expertise helps ensure patients and families receive compassionate, seamless care when they need it most. As a Hospice Intake & Admissions RN, you are often the first point of contact on a patient’s hospice journey. Your work sets the tone—providing comfort, clarity, and reassurance while guiding patients and families through the transition to hospice care. This role offers a dynamic schedule, combining time in our central intake office in Albany, and field visits to patients in homes, nursing homes, and assisted living communities across the region.

Requirements

Active New York State RN license
ASN required
Valid driver’s license and reliable transportation
Strong communication skills—you connect easily with patients, families, and clinical teams
Clear, concise documentation abilities
Comfort with computers and electronic health records
Excellent organization and time management skills

Nice To Haves

BSN preferred
Home care experience (1+ year) strongly preferred
Hospice or end-of-life care experience is a plus

Responsibilities

Serve as a key liaison between patients, families, providers, and our interdisciplinary team
Review and coordinate referrals to determine hospice eligibility in collaboration with physicians and care teams
Meet patients where they are—at home or in care facilities—to complete initial assessments
Initiate care plans, including ordering medications, medical equipment, and supportive services
Partner with social workers, chaplains, aides, therapists, and volunteers to provide holistic care
Ensure a smooth, supportive handoff to the Hospice Case Manager
