About The Position

Allay Hospice is seeking a passionate, dedicated Hospice Admissions RN to join our team! Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. As a Hospice Admissions RN, you will visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care. You will explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents. You will also obtain medical history, current medications, and symptoms, and perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines. Additionally, you will coordinate the start of services (nursing, aides, equipment, medications) and collaborate with the interdisciplinary team, maintaining accurate and timely documentation.

Requirements

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Nice To Haves

• 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
