About The Position

Suncrest At Brighton/Suncrest Hospice aims to redefine hospice care by offering exceptional service and care to patients. This is achieved by increasing staff to patient ratios, which allows for more clinical visits and lower clinician caseloads. The organization is CHAP certified. This role is for a Registered Nurse (RN) committed to providing high-quality care to patients and their families.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
• Ability to work collaboratively within a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to work independently on a day-to-day basis.
• Proficiency in phone, email, and written communication.
• Understanding of hospice rules and regulations.
• Strong time management skills to manage visits, documentation, and meetings.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area.

• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
• Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)

Responsibilities

• Provide care and services for a group of patients.
• Visit patients in their residences, including private homes, Skilled Nursing Facilities, and assisted living facilities.
• Serve as a contributing member of the interdisciplinary team.
• Provide expertise regarding psych-social assessments, interventions, and knowledge of community resources.
• Work in a Case Management, Admissions, or Resource role.
