Vitality Hospice & Palliative Care is seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our clinician-owned and operated hospice agency. Requirements

Active Registered Nurse (RN) license (required)
Valid driver’s license, reliable transportation, and auto liability insurance
Strong communication, organizational, and interpersonal skills

Nice To Haves

1+ year of skilled nursing experience preferred
Hospice or admissions experience preferred (not required)
Willingness to travel within assigned service areas

Responsibilities

Complete comprehensive hospice admissions and patient/family assessments
Ensure accurate documentation and appropriate care planning
Provide direct patient care as needed, including medication administration and treatments
Collaborate with physicians and the interdisciplinary team to develop individualized plans of care
Serve as a liaison between patients, families, and healthcare providers
Educate patients and caregivers on disease progression, symptom management, and hospice services
