About The Position

Glen Cove Center for Nursing and Rehabilitation is looking for an RN Admissions Nurse. The Admission Nurse is responsible for admitting all new patients to the facility while ensuring the nursing operational standards of the facility are met. This role requires strong communication and assessment skills, the ability to plan and evaluate nursing care, and collaboration with the interdisciplinary team. The nurse will complete all admission-related paperwork, including assessments, care plans, and obtaining necessary consents.

Ability to assess, plan, implement and evaluate nursing care according to the individualized needs.
Comply with all state requirements and facility policies, procedures and standards of conduct.

Responsibilities

Admit all new patients to the facility.
Ensure nursing operational standards are met.
Assess, plan, implement, and evaluate nursing care according to individualized needs.
Communicate and collaborate with the interdisciplinary team and/or physicians.
Complete all paperwork for the Nursing Admission Assessment.
Enter all data into Electronic Medical Record.
Obtain orders from MD and ensure accuracy of Admission Orders in EMR.
Complete the Admission Nursing Note.
Complete the Fall Risk Assessment and interventions care plans as indicated.
Complete the Pressure Ulcer Risk Assessment / Braden Scale and ensure preventative measures are implemented if high risk.
Obtain consents: Pneumovax / Flu and place in the patients’ chart.
Ensure Initial/Interim Care plans are put in place for Fall Risk, Skin Alt-Potential, Pain-specific resident conditions (pressure ulcer, post-op, infection, etc).
Administer 1st dose of PPD within 24 hrs and record on Immunization Record and Med sheet.
Ensure all paperwork for the admission is complete and signed off on.
Ensure that all follow-up consults are created for the Unit Secretary to schedule in a timely fashion.
Report to the RN Supervisor on duty.
