About The Position

The RN, RPM Analyst is a clinically sharp, action-oriented registered nurse who sits at the intersection of technology and patient care. This role monitors inbound subjective and objective data from remote patient monitoring devices, rapidly triages escalations, and engages patients, providers, and care team members to prevent clinical deterioration and reduce avoidable exacerbations. Requirements

  • Active Registered Nurse (RN) license in good standing in Georgia and a multi-state compact license
  • Minimum of 5 years of clinical nursing experience, with a strong foundation in assessment, triage, and care coordination for patients with chronic or complex conditions.
  • Prior experience in telemonitoring, phone triage, telehealth nursing, or remote patient monitoring as a principal function of the role.
  • Demonstrated experience and ability to rapidly evaluate, analyze, and synthesize both subjective patient-reported information and objective biometric data to inform clinical decision-making.
  • Exceptional written communication skills, with the ability to produce clear, concise, and clinically accurate documentation and care team correspondence under time pressure.
  • Strong proficiency with electronic health records (EHR) and comfort navigating digital health platforms, data dashboards, and remote monitoring technology.
  • Demonstrated urgency, reliability, and responsiveness — this role operates in a time-sensitive environment where delayed action has direct patient safety implications.
  • Effective verbal communication and interpersonal skills for engaging patients, families, and multidisciplinary care teams across telephonic and virtual settings.
  • Ability to continuously monitor and respond to patient data alerts and escalations throughout the work shift with a high degree of vigilance and clinical accuracy.
  • Must be able to respond to critical patient alerts within defined response time thresholds established by clinical protocols, without exception.

Nice To Haves

  • Clinical background in cardiology, pulmonology, endocrinology, or another specialty with high chronic disease burden relevant to RPM populations (e.g., CHF, COPD, hypertension, diabetes).
  • Familiarity with CMS RPM coding and billing requirements (CPT codes 99453, 99454, 99457, 99458) and their documentation implications.
  • Experience working in a value-based care, managed care, or ACO environment with population health accountability.
  • Bilingual in English and Spanish or another language reflective of the patient population served.

Responsibilities

  • Monitoring inbound remote patient monitoring (RPM) data streams in real time, including vital signs, weight, blood glucose, blood pressure, pulse oximetry, and patient-reported symptoms, to identify clinical changes requiring intervention.
  • Triaging patient escalations using clinical judgment, established protocols, and patient-specific care plans, escalating to the supervising provider or care team when warranted.
  • Initiating timely outbound patient engagement via phone, secure message, or telehealth to assess subjective status, provide clinical coaching, and determine next steps in care.
  • Collaborating with ordering providers, specialists, care coordinators, and interdisciplinary care team members to communicate clinical findings and coordinate timely care plan adjustments.
  • Evaluating trending data patterns across individual patients and assigned panels to proactively identify early signs of clinical deterioration and intervene before acute exacerbation occurs.
  • Documenting all clinical interactions, assessments, escalations, and interventions accurately and thoroughly within the electronic health record (EHR) and RPM platform in compliance with regulatory and organizational standards.
  • Maintaining awareness of CMS RPM billing requirements, including time tracking, touchpoint documentation, and monthly threshold compliance to support accurate and timely reimbursement.
  • Contributing to quality improvement efforts by identifying protocol gaps, reporting alert fatigue patterns, and recommending threshold adjustments based on clinical observation.
  • Educating patients on device usage, data submission expectations, and self-management techniques tailored to their chronic condition(s), fostering engagement and adherence to the RPM program.
  • Producing thorough, timely, and clinically precise written documentation for every patient interaction in compliance with HIPAA, organizational policy, and applicable state nursing practice standards.
