Description Summary: The Remote Patient Monitoring Technician (RPM Tech) supports The Guthrie Clinic’s Remote Patient Monitoring program with successful patient transitions from hospital to home, chronic disease management, care coordination, and patient triage. The RPM Tech partners with the Care Transitions Nurse Navigators and other RPM-associated team members. Responsible for linking mobile resources together, integrating and coordinating them to allow for an appropriate response to patient needs. Telephonic patient triage is provided following established evidence-based protocols to assist in navigating care across the health care continuum. Demonstrates appropriate communication skills and interactions with patients, family members, and other members of the patient care team. Assists with the organization and completion of simple projects under the supervision of Pulse Center leadership. Experience: Prior clinical or direct patient care experience preferred but not required. Proficiency with Microsoft programs (Excel, Word, Outlook, Teams, etc.) preferred. Professional communication and teamwork required. Education: High School education or GED required. Must be comfortable using computers and learning new technology. Essential Functions: Coordinate deployment of home-based care services as appropriate (home health, paramedicine, urgent/emergent EMS response) with RPM team members. Access, navigate, and document as appropriate in the EMR System. Maintain patient privacy per HIPAA guidelines. Act as a point of contact for patients enrolled in the Chronic Care Management program. Monitor alerts and check for missing data in a timely manner Assist patients with set up and troubleshooting for remote monitoring equipment Receive and relay patient questions/concerns, and coordinate care as appropriate per established evidence-based protocols. Schedule follow up appointments for the patient as needed. Provide feedback to the appropriate care coordination staff members regarding patient interactions. Collaborate with Primary Care, Care Coordination, EMS crews, Pulse Center, and other specialists to meet patient needs. Demonstrate adherence to Guthrie values of Patient Centeredness, Teamwork, and Excellence. Other Duties: Participates in performance improvement activities to enhance service and patient care. Seek supervision and guidance when appropriate.
