This role is responsible for coordinating and facilitating the education of the outpatient diabetes patient population, acting as a liaison between patients, families, and medical staff. The position involves organizing and scheduling patients for the Outpatient Diabetes Self-Management Program, providing consultations on diabetes therapy, and encouraging family participation. The individual will instruct outpatient groups and one-on-one sessions, document outpatient education in patient records, and develop teaching tools and aids. This role also serves as a resource and clinical expert for identified diabetes inpatients and hospital staff, assists with educating newly diagnosed patients, assesses needs of complicated diabetes patients, and reinforces education as requested. Additionally, the position involves making multi-disciplinary referrals, consulting with Medical Staff, providing training for monitoring equipment, and ongoing education for hospital staff regarding diabetes management changes.
