About The Position
This role involves phone triage for patients, requiring the application of the nursing process to assess, plan, implement, and evaluate patient care. The Registered Nurse will collaborate with physicians and other healthcare team members to coordinate procedures and treatments, utilizing leadership skills and clinical judgment. Responsibilities include assessing patients' physiological and psychosocial health status, identifying patient outcomes, formulating care plans, implementing and evaluating care, and collaborating with the care team. The role also involves performing specific nursing procedures such as wound care and medication administration, as well as participating in patient and family education.
Requirements
- Graduate from an accredited School of Nursing. Associate’s Degree in Nursing, required.
- Minimum 2 years of professional nursing experience in the related specialty.
- Current license to practice as a Registered Professional Nurse in New York State, required.
- BLS required, plus specialized certifications as needed.
Nice To Haves
- Bachelor’s Degree in Nursing, preferred.
- 3-5 years of ICU, Med Surg, Operating Room, PACU, Cath Lab, Primary Care, or Cardiology preferred
- Phlebotomy, Injection Administration, IV insertion, EKG, Vital signs, Phone/In Person Triage, Basic Wound Care.
Responsibilities
- Assesses the patient’s physiologic health status
- Assesses patient’s psychosocial health status
- Identifies patient outcome
- Formulates the plan of care and establishes the priorities necessary to achieve expected outcome
- Implements the plan of care
- Evaluates care provided for patient outcome
- Collaborates with other care team members in planning and carrying out treatment regimen; provides direction to other members of the care team
- Performs grade I-IV Decubitus Care
- Administers medications correctly and safely
- Participates in patient and family education
- Performs related duties, as required.
