TRU Community Care is looking for a phone triage nurse. This nurse will work with our afterhours and triage team, taking in calls from patients and collaborating with visit nurses on patient needs as necessary. The Triage Registered Nurse performs in a supportive role to the Care Team by answering calls coming into the agency, with response to the telephone call by delegating tasks. Monitors and responds to triage calls within the agency specific time limits. Evaluates and assesses patients’ health care needs based on patient’s signs and symptoms utilizing established triage protocols and guidelines. Obtains appropriate input and direction from the provider as needed to determine the best clinical course of action and recommendations for specific patient health care needs. May direct them to emergency room, urgent care, provide home care advice, or arrange for an in-home nursing visit. Utilize triage protocols appropriately.
